2026 BMCH Photography Registration & Permission Form

Please complete this form to give permission for your child to participate in the upcoming BMCH kinder photography sessions.

There is no cost to register and no obligation or commitment to purchase photographs. Registration simply provides the permissions needed for your child to be included in the photography sessions. Please read each permission option carefully and select the permissions you are comfortable providing.

You can give permission for group photographs, individual photographs and marketing photographs separately. Giving permission for one does not automatically give permission for the others.
